Patient Care Coordination

Patients and familes are struggling to receive care in a coordinate manner. Appointments are made months in advance, you have 5 minutes of a provider's time and you leave having to make other appointments causing a delay in diagnosis.   How can we better coordinate the care during one visit vs. multiple visits?
