Capability Scorecard

Capability refers to an analysis that determines how well a process can deliver to customer requirements. Scorecards list the capability of key process and/or product requirements.

Cause & Effect Matrix

The C&E Matrix is used to quantitatively relate the impact of inputs (X's) on the customer outputs (Y's) of a given process. The C&E Matrix template provides a framework for this evaluation.

Cause & Effect Project Prioritization Matrix

Project prioritization is one of the most important aspects of Lean Six Sigma. This C&E Matrix template can be used to prioritize projects based on quantitative evaluation.

Contingency Table Template

Contingency Tables are used to evaluate relationships between two or more attribute or categorical factors. This Excel-based template provides formulas for expected values and chi-square values.

  • Lean Six Sigma
  • Performance Excellence
Control Plan Template

The Control Plan is a summary of all control activities for a process. It is intended to be used after the project is handed off to the process owner in order to sustain the gains of the Lean Six Sigma project. This template provides a best practice framework for capturing process control activities.

Data Collection Plan Template

The Data Collection Plan defines what type of data will be collected during a Six Sigma project. This template provides a framework for documenting the what, where and how much of your data collection efforts.

Data Validation Questions In Lieu of Measurement Systems Analysis

If it just isn’t feasible to conduct a traditional MSA, then at the very least, these questions should be applied.  You should document the answers to this checklist as part of your project documentation / Final Report.

DOE Outline

A DOE, or Design of Experiment, is a test in which purposeful changes are made to the input variables in a process so that changes in the output can be observed. The DOE Outline provides a framework for students to create a meaningful summary of their completed DOE and how it contributed to the project.

DOE Plan

A DOE, or Design of Experiment, is a test in which purposeful changes are made to the input variables in a process so that changes in the output can be observed. The DOE Plan is a document that students can use to plan experiments and track results.

Final Report Deliverables Matrix

Tracking project deliverables is an important way to ensure a Lean Six Sigma project is on target.  This worksheet lists the deliverables for the Define, Measure, Analyze, Improve and Control phases of a Lean Six Sigma project.
